We are seeking a passionate and experienced Director of Social Services to lead our Social Services department and support our mission.
Director of Social Services Position Summary
The Director of Social Services is responsible for planning, organizing, and directing the overall operation of the Social Services department in accordance with regulatory guidelines, facility policies, and best practices in resident care. This leader works closely with residents, families, interdisciplinary teams, and external partners to ensure psychosocial needs are met and smooth transitions of care occur.
Director of Social Services Key Responsibilities:
• Lead, supervise, and support Social Services staff, including onboarding, training, and ongoing development.
• Complete psychosocial assessments, care plans, and documentation in accordance with state, federal, and facility standards.
• Advocate for residents’ rights and needs, ensuring resident-focused care approaches.
• Facilitate admissions and discharges, including coordination with hospitals, families, insurance providers, and community resources.
• Provide counseling, crisis intervention, and support for residents and families.
• Participate actively as part of the interdisciplinary team (IDT) during care plan meetings, case reviews, and quality improvement processes.
• Maintain compliance with all regulatory requirements, including CMS, state regulations, and survey preparedness.
• Develop and maintain positive relationships with residents, families, staff, and community partners.
• Assist residents with accessing community support services, financial resources, and advanced directive planning.
Director of Social Services Qualifications:
• Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred.
• Current state Social Work license or eligibility per state regulations.
• Minimum 2–3 years of social work experience in a healthcare or skilled nursing facility setting; leadership experience preferred.
• Strong understanding of SNF regulations, discharge planning, and psychosocial assessments.
• Excellent communication, organization, and interpersonal skills.
• Ability to work collaboratively and compassionately with residents, families, and teams.
• Demonstrated leadership ability and commitment to resident-centered care.
