This role plays a vital part in supporting residents and families throughout their stay and ensuring high-quality, person-centered care.
Social Worker Responsibilities include: - Providing psychosocial assessments and ongoing support to residents and families
- Assisting with discharge planning and coordination of community resources
- Participating in care plan meetings and interdisciplinary team discussions
- Supporting residents' rights, emotional well-being, and care transitions
- Maintaining accurate and timely documentation in accordance with regulations
Social Worker Qualifications: - BSW or MSW required
- Long-term care or SNF experience preferred
- Strong communication, organizational, and interpersonal skills
- Knowledge of Medicare, Medicaid, and discharge planning processes
