The Monarch at Brooklyn Rehabilitation & Nursing Center is seeking a compassionate and dedicated Social Worker to join our interdisciplinary care team. This role plays a vital part in supporting residents and families throughout their stay and ensuring high-quality, person-centered care.
Social Worker Responsibilities include:

Providing psychosocial assessments and ongoing support to residents and families

Assisting with discharge planning and coordination of community resources

Participating in care plan meetings and interdisciplinary team discussions

Supporting residents’ rights, emotional well-being, and care transitions
-
Maintaining accurate and timely documentation in accordance with regulations
Social Worker Qualifications:

BSW or MSW required
-
Long-term care or SNF experience preferred
-
Strong communication, organizational, and interpersonal skills

Knowledge of Medicare, Medicaid, and discharge planning processes
