Wound Care Nurse / Infection Preventionist (RN)
Job Overview:
Water’s Edge Rehab & Nursing Center at Port Jefferson is seeking a skilled and dedicated Wound Care Nurse / Infection Preventionist (RN) to lead wound care programs and support infection prevention and control efforts within our skilled nursing facility. This dual-focus role is vital to enhancing resident outcomes, promoting best practices, and ensuring compliance with clinical standards and regulatory requirements.
Wound Care Nurse / Infection Preventionist (RN) Key Responsibilities:

Lead and coordinate wound care assessment, treatment planning, and monitoring for residents

Oversee the facility’s infection prevention and control program, including surveillance, education, and reporting

Collaborate with interdisciplinary teams (nursing, therapy, physicians) to support resident care plans

Monitor infection trends and provide data analysis to guide prevention strategies

Ensure compliance with CMS, DOH, and IP standards

Provide staff education on wound care protocols and infection control best practices

Maintain accurate documentation and ensure thorough clinical records

Participate in quality improvement initiatives and survey readiness activities
Wound Care Nurse / Infection Preventionist (RN) Qualifications:

Current Registered Nurse (RN) license in New York State

Experience in wound care and infection prevention in a skilled nursing, long-term care, or acute care setting

Strong knowledge of clinical wound care standards and infection prevention protocols

Excellent communication, organizational, and leadership skills

Ability to work collaboratively with clinical and administrative teams
