Admissions Nurse (RN) – Skilled Nursing Facility
We are seeking a highly organized and clinically skilled Admissions Nurse to join our Skilled Nursing Facility team. The Admissions Nurse plays a critical role in ensuring smooth and timely admissions by coordinating clinical reviews, completing assessments, and collaborating with hospital partners, families, and interdisciplinary teams. This position is ideal for a nurse who excels in communication, documentation, and care coordination.
Admissions Nurse (RN) Key Responsibilities

Coordinate and manage the full admissions process for new residents

Review hospital referrals, medical records, and clinical documentation for appropriateness of admission

Complete admission assessments and ensure accuracy of clinical data

Collaborate with hospitals, discharge planners, families, and internal departments

Verify insurance authorizations and ensure compliance with admission criteria

Communicate effectively with nursing, therapy, social services, and administrative teams

Ensure all admissions comply with federal, state, and facility regulations

Participate in tours and serve as a clinical resource for prospective residents and families

Maintain accurate and timely documentation in the EMR system
Admissions Nurse (RN) Qualifications

Active RN license in good standing (NY)

Minimum 2–3 years of nursing experience, preferably in a SNF, sub-acute, or acute care setting

Admissions, utilization review, case management, or MDS experience strongly preferred

Strong clinical assessment and decision-making skills

Excellent communication, organization, and time-management abilities

Knowledge of Medicare, Medicaid, and managed care guidelines is a plus

Ability to work collaboratively in a fast-paced environment
Compensation & Benefits
Salary: Up to $125,000 per year
