MDS Assessor (RN) - Skilled Nursing Facility A well-established
Skilled Nursing Facility (SNF) is seeking an experienced and detail-oriented
MDS Assessor (RN) to join our clinical team. This is an excellent opportunity for a motivated nurse who is passionate about quality resident care, regulatory compliance, and accurate clinical reimbursement.
The ideal candidate will have strong knowledge of the
MDS process, care planning, and PDPM, and will work closely with the interdisciplinary team to ensure timely and accurate assessments.
MDS Assessor (RN) Responsibilities - Complete and coordinate Minimum Data Set (MDS) assessments in accordance with federal and state regulations
- Conduct Care Area Assessments (CAAs) and participate in the development of comprehensive care plans
- Ensure accurate and timely submission of MDS assessments to CMS
- Collaborate with nursing, therapy, and interdisciplinary teams to gather clinical documentation
- Monitor and ensure compliance with PDPM guidelines and reimbursement requirements
- Participate in Medicare meetings and care plan meetings
- Assist with quality measures, clinical audits, and regulatory compliance initiatives
- Maintain current knowledge of CMS regulations and MDS updates
MDS Assessor (RN) Qualifications - Active RN license in good standing
- MDS experience in a Skilled Nursing Facility preferred
- Strong knowledge of PDPM, Medicare/Medicaid guidelines, and care planning
- Excellent attention to detail and organizational skills
- Ability to work collaboratively with an interdisciplinary team
- Strong communication and documentation skills
