Salary: Up to $90,000 per year

Director of Social Services Water's Edge Rehab & Nursing Center at Port Jefferson is seeking an experienced and compassionate
Director of Social Services to lead our social services department. This leadership role is responsible for overseeing the psychosocial care of our residents and ensuring compliance with federal and state regulations while supporting residents and their families throughout their stay.
The ideal candidate will be a
strong advocate for residents, an effective communicator, and an experienced professional who thrives in a collaborative healthcare environment.
Director of Social Services Key Responsibilities - Oversee and manage the Social Services Department within the facility
- Conduct psychosocial assessments for residents upon admission and throughout their stay
- Develop and implement individualized care plans addressing residents' psychosocial needs
- Provide counseling and emotional support to residents and their families
- Coordinate discharge planning and ensure smooth transitions to home or other care settings
- Participate in interdisciplinary care plan meetings
- Ensure compliance with state and federal regulations related to social services in long-term care
- Assist residents and families with community resources and support services
- Address resident concerns and advocate for their needs to improve quality of life
Director of Social Services Qualifications - Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred
- Bachelor's degree in Social Work required (Master's preferred)
- Prior experience in a skilled nursing facility or long-term care setting strongly preferred
- Previous supervisory or leadership experience is a plus
- Strong knowledge of care planning, discharge planning, and regulatory compliance
- Excellent interpersonal, organizational, and communication skills
