DescriptionWe are seeking a dedicated and experienced
Registered Nurse (RN) and/or
Licensed Practical Nurse (LPN) to join our team as a
Charge Nurse. In this role, you will support the Subacute Unit Manager in driving positive patient outcomes and delivering high-quality care.
Key Responsibilities:
- Assist with oversight of patient care and outcomes
- Provide support with admissions and discharges
- Perform and manage wound care
- Monitor and support cardiac patients
- Communicate effectively with patients and families, including outreach as needed
Qualifications: - Prior experience in subacute or skilled nursing preferred
- Strong clinical skills in wound care and cardiac care
- Experience with admissions, discharges, and care coordination
- Commitment to patient-centered care and teamwork
