About the Role The Phoenix Rehabilitation and Nursing Center is seeking a compassionate and motivated
Bilingual Social Worker (Spanish/English) to join our interdisciplinary care team. This role plays a vital part in supporting residents and families with psychosocial needs, care planning, and discharge coordination in a skilled nursing environment.
The ideal candidate is empathetic, culturally competent, well-organized, and passionate about advocating for residents.
Social Worker Bilingual (Spanish/English) Key Responsibilities - Conduct psychosocial assessments and develop care plans
- Provide counseling and emotional support to residents and families
- Serve as liaison between residents, families, and interdisciplinary team
- Facilitate discharge planning and coordinate community resources
- Participate in admissions and resident orientation
- Address resident and family concerns and grievances
- Maintain accurate documentation in the EMR system
- Participate in care conferences and family meetings
- Ensure compliance with CMS and state regulations
Social Worker Bilingual (Spanish/English) Qualifications - Bachelor's or Master's degree in Social Work (BSW or MSW)
- Bilingual in Spanish and English (required)
- At least 1 year experience in a healthcare or long-term care setting preferred
- Strong communication, organizational, and interpersonal skills
- Knowledge of discharge planning and community resources
- Experience with PointClickCare or similar EMR preferred
Salary: Up to $70,000 per year (based on experience)
