Social Worker Responsibilities- Conduct resident assessments to identify social, emotional, and psychosocial needs
- Develop, implement, and monitor individualized care plans in collaboration with the healthcare team
- Provide counseling, advocacy, and support to residents and their families
- Assist with discharge planning, ensuring safe and appropriate transitions to home or other care settings
- Collaborate with interdisciplinary team members to promote resident-centered care
- Maintain accurate documentation and records in compliance with state and federal regulations
- Serve as a resource for community services and benefits for residents and families
Social Worker Qualifications- Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program
- Valid New York State Social Work license (LMSW or LCSW preferred)
- Experience in skilled nursing or long-term care preferred
- Strong communication, organizational, and interpersonal skills
- Ability to work collaboratively as part of a multidisciplinary team
- Compassionate, proactive, and resident-focused
$26 and hour
