Green Hills Center for Rehabilitation and Healing is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. In this role, you will play a vital part in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and enhancing overall quality of life within our skilled nursing environment.
Key Responsibilities: - Provide emotional support and counseling to residents and their families
- Conduct psychosocial assessments and develop individualized care plans
- Coordinate discharge planning to ensure smooth transitions of care
- Advocate for residents' rights and overall well-being
- Collaborate with nursing, therapy, and medical teams to support holistic care
- Assist residents and families with community resources and services
- Maintain accurate and timely documentation in accordance with regulatory standards
- Participate in care plan meetings and interdisciplinary team discussions
Qualifications: - Bachelor's Degree in Social Work (BSW) required; Master's Degree (MSW) preferred
- Prior experience in a skilled nursing facility or healthcare setting is a plus
- Strong understanding of discharge planning and care coordination
- Excellent communication and interpersonal skills
- Ability to handle sensitive situations with professionalism and empathy
- Knowledge of state and federal regulations preferred
