Admissions Coordinator to join our team. This role is ideal for someone who thrives in a fast-paced healthcare environment and is passionate about providing a seamless admissions experience for residents and their families.
Position Overview The Admissions Coordinator is responsible for managing the intake and admission process, serving as a key point of contact for referral sources, hospitals, families, and internal departments. This individual plays a vital role in ensuring efficient transitions into the facility.
Key Responsibilities - Coordinate all aspects of the admissions process from initial referral through resident admission
- Communicate with hospitals, case managers, families, and referral sources
- Review clinical and financial documentation to ensure eligibility and appropriateness for admission
- Verify insurance coverage and obtain necessary authorizations
- Collaborate with interdisciplinary teams to facilitate smooth and timely admissions
- Maintain accurate records and census tracking
- Provide facility tours to prospective residents and families
- Assist in developing and maintaining strong relationships with referral sources
- Ensure compliance with all applicable regulations and facility policies
Qualifications - Prior experience in admissions, intake, or healthcare coordination preferred
- Experience in a skilled nursing facility or healthcare setting strongly preferred
- Knowledge of insurance verification, including Medicare, Medicaid, and managed care
- Strong communication, customer service, and organizational skills
- Ability to multitask and work efficiently in a fast-paced environment
- Proficiency with electronic medical records (EMR) systems; PCC experience a plus
