The Paramount at Somers Rehabilitation and Nursing Center is seeking an experienced and compassionate Registered Dietitian (RD) to join our interdisciplinary healthcare team. The ideal candidate will be responsible for assessing residents' nutritional needs, developing individualized nutrition care plans, and ensuring the highest standards of nutritional care and regulatory compliance.

Responsibilities - Complete nutritional assessments and develop individualized care plans for residents
- Monitor residents' nutritional status and document progress notes as required
- Provide nutrition counseling and education to residents and families
- Collaborate with physicians, nursing staff, and interdisciplinary team members regarding resident care
- Review and approve therapeutic diets and menu plans
- Ensure compliance with federal, state, and facility dietary regulations
- Participate in care plan meetings and quality assurance initiatives
- Monitor food service operations for compliance with sanitation and dietary standards
- Assist with survey readiness and regulatory compliance activities
- Provide support and guidance to dietary department staff as needed
Qualifications - Current Registered Dietitian (RD) credential required
- Previous experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred
- Strong knowledge of clinical nutrition, dietary regulations, and care planning
- Excellent communication, organizational, and interpersonal skills
- Ability to work collaboratively within an interdisciplinary healthcare team
