The Legacy at Boca Raton Rehabilitation and Nursing Center is seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team. The ideal candidate will be responsible for evaluating residents, developing individualized treatment plans, and providing high-quality therapy services to help residents achieve their highest level of independence and functional mobility.

Responsibilities - Evaluate residents and develop individualized physical therapy treatment plans
- Provide therapeutic exercises, mobility training, balance training, and pain management interventions
- Monitor resident progress and adjust treatment plans as needed
- Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal resident outcomes
- Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements
- Educate residents and families regarding therapy programs and treatment goals
- Ensure compliance with all federal, state, and facility policies and regulations
- Participate in care planning meetings and discharge planning processes
- Maintain a safe and supportive therapy environment
Qualifications - Active Florida Physical Therapist (PT) license in good standing
- Previous experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal abilities
- Ability to work collaboratively within an interdisciplinary team
