Director of Social Services to lead our Social Services Department within our Skilled Nursing Facility.
The ideal candidate will be passionate about advocating for residents and their families while ensuring exceptional psychosocial support throughout the continuum of care. This leadership role is responsible for overseeing social services operations, supporting discharge planning, ensuring regulatory compliance, and enhancing the overall resident experience.
Responsibilities- Direct and oversee all functions of the Social Services Department.
- Complete psychosocial assessments and develop individualized care plans.
- Coordinate discharge planning and facilitate safe transitions of care.
- Provide counseling and emotional support to residents and their families.
- Assist residents and families with advance directives, healthcare proxies, and community resources.
- Participate in interdisciplinary care plan meetings and quality improvement initiatives.
- Advocate for resident rights and promote resident satisfaction.
- Ensure compliance with all federal, state, and local regulations governing social services in long-term care.
- Maintain accurate documentation and required assessments.
- Investigate and address resident concerns, grievances, and family issues.
- Collaborate with nursing, rehabilitation, admissions, and other departments to support resident-centered care.
- Prepare for and participate in state and federal surveys.
Qualifications- Bachelor's degree in Social Work, Human Services, Psychology, or a related field required.
- Master's degree in Social Work preferred.
- Current state licensure or certification, if required by state regulations.
- Minimum of two years of social services experience in a Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, or healthcare setting preferred.
- Previous supervisory or management experience preferred.
- Strong knowledge of discharge planning, care coordination, and long-term care regulations.
- Excellent communication, organizational, and interpersonal skills.
- Ability to manage multiple priorities in a fast-paced environment.
