Social Worker to join our interdisciplinary team. In this role, you will advocate for our residents while providing emotional support, discharge planning, and care coordination to enhance their quality of life.

Responsibilities:- Complete psychosocial assessments for residents upon admission and as needed
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Provide counseling and emotional support to residents and their families
- Coordinate discharge planning and transitions of care
- Assist residents and families with community resources, financial assistance, and long-term care planning
- Facilitate resident and family meetings as needed
- Ensure compliance with all federal, state, and facility regulations
- Maintain accurate and timely documentation
- Participate in care plan meetings and quality improvement initiatives
Qualifications:- Bachelor's Degree in Social Work (BSW) required; Master's Degree (MSW) preferred
- Previous experience in a skilled nursing, rehabilitation, or long-term care setting preferred
- Strong communication, organizational, and interpersonal skills
- Knowledge of discharge planning, resident rights, and long-term care regulations
- Ability to work collaboratively as part of an interdisciplinary team
