Social Worker to join our interdisciplinary team. This is an excellent opportunity for a caring professional who is passionate about supporting residents and their families while helping them navigate their healthcare journey in a skilled nursing and rehabilitation setting.
As a Social Worker, you will play a vital role in promoting residents' emotional well-being, coordinating services, and ensuring a positive experience throughout their stay.
Responsibilities- Complete psychosocial assessments for new admissions and ongoing resident evaluations.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide emotional support, counseling, and crisis intervention for residents and their families.
- Coordinate discharge planning to ensure safe and successful transitions.
- Assist residents and families with community resources, financial assistance, and advance care planning.
- Advocate for resident rights and promote dignity, independence, and quality of life.
- Participate in interdisciplinary care plan meetings and family conferences.
- Maintain accurate documentation in accordance with CMS, state, and facility requirements.
- Collaborate with nursing, rehabilitation, admissions, and medical staff to support resident-centered care.
- Assist with resident adjustment, conflict resolution, and grievance management.
- Support regulatory compliance and survey readiness.
Qualifications- Bachelor's degree in Social Work or a related Human Services field required.
- Social Work experience in a Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, or healthcare setting preferred.
- Knowledge of discharge planning, resident rights, and CMS regulations preferred.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work collaboratively within a multidisciplinary team.
