Director of Social Services to lead our Social Services Department. This is an outstanding opportunity for a dedicated professional who is passionate about resident advocacy, family support, and delivering exceptional care in a skilled nursing and rehabilitation environment.
As the Director of Social Services, you will oversee all social services operations, ensuring residents receive the emotional, psychosocial, and discharge planning support needed to enhance their quality of life and achieve the best possible outcomes.
Responsibilities- Oversee the daily operations of the Social Services Department.
- Complete comprehensive psychosocial assessments and develop individualized care plans.
- Supervise, mentor, and support the Social Services team.
- Coordinate discharge planning to ensure safe and successful transitions.
- Provide counseling, crisis intervention, and emotional support to residents and their families.
- Lead care plan meetings and family conferences as part of the interdisciplinary team.
- Advocate for resident rights while promoting dignity, respect, and independence.
- Connect residents and families with community resources and support services.
- Maintain accurate documentation in compliance with CMS, New York State, and facility regulations.
- Ensure the department is survey-ready and supports regulatory compliance.
- Collaborate with nursing, rehabilitation, admissions, and medical staff to optimize resident outcomes.
- Participate in quality assurance and performance improvement initiatives.
Qualifications- Bachelor's Degree in Social Work (BSW) required; Master's Degree (MSW) preferred.
- Current New York Social Work license, where applicable.
- Previous leadership experience in a Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, or healthcare setting preferred.
- Strong knowledge of discharge planning, resident rights, and CMS regulations.
- Excellent leadership, communication, and organizational skills.
- Ability to manage multiple priorities in a fast-paced healthcare environment.
