motivated and organized Admissions Coordinator to join our team. This is an excellent opportunity for someone who enjoys working with patients and families, coordinating admissions, and being part of a fast-paced healthcare environment.
What You'll Do- Coordinate and facilitate the admissions process from referral through arrival.
- Review referral information and coordinate with hospitals, physicians, families, and referral sources.
- Ensure admission paperwork and documentation are complete and accurate.
- Communicate admission details with nursing, social services, billing, and other departments.
- Maintain accurate census and admission records.
- Build and maintain strong relationships with hospitals, case managers, and community referral sources.
- Respond promptly to inquiries and help ensure a smooth transition for residents entering the facility.
- Assist with insurance and authorization-related information as needed.
- Provide excellent customer service to residents and their families.
What We're Looking For- Previous experience in SNF admissions, healthcare admissions, or a related healthcare setting preferred.
- Strong communication and organizational skills.
- Ability to multitask and work effectively in a fast-paced environment.
- Professional, compassionate, and service-oriented approach.
- Strong attention to detail and follow-through.
- Experience with healthcare documentation and insurance/authorization processes is a plus.
