What You'll Do- Provide high-quality nursing care to residents in accordance with individualized care plans.
- Administer medications and treatments as ordered.
- Monitor residents' conditions and report changes to the RN and medical team.
- Complete accurate and timely clinical documentation.
- Work closely with CNAs, RNs, physicians, and other members of the interdisciplinary team.
- Assist with assessments, care planning, and resident needs.
- Promote resident safety, dignity, comfort, and quality of life.
- Communicate effectively with residents and their families.
What We're Looking For- Current New York State LPN license.
- Previous nursing experience preferred; skilled nursing experience is a plus.
- Strong clinical, communication, and organizational skills.
- Compassionate and resident-focused approach.
- Ability to work effectively as part of a team.
