Clinical Nutrition Manager to join our team. This is an excellent opportunity for a motivated nutrition professional who is passionate about providing high-quality, resident-centered care in a skilled nursing and rehabilitation setting.
The Clinical Nutrition Manager will oversee the clinical nutrition program and work closely with the interdisciplinary team to ensure residents receive individualized nutrition care that supports their health, recovery, and overall quality of life.
Responsibilities- Oversee the clinical nutrition services and ensure delivery of high-quality, resident-centered nutrition care.
- Complete and oversee comprehensive nutritional assessments for residents.
- Develop individualized nutrition care plans based on residents' medical conditions, dietary needs, preferences, and goals.
- Monitor residents at nutritional risk, including those experiencing weight loss, poor intake, wounds, or other nutrition-related concerns.
- Collaborate with Nursing, Physicians, Speech Therapy, Rehabilitation, and other interdisciplinary team members.
- Participate in care plan meetings and provide clinical nutrition recommendations.
- Monitor weights, meal intake, laboratory values, and other relevant clinical indicators.
- Make appropriate recommendations regarding therapeutic diets, supplements, and nutrition interventions.
- Communicate effectively with residents, families, nursing staff, and medical providers regarding nutrition-related concerns.
- Supervise, support, and provide direction to dietary and nutrition team members as appropriate.
- Ensure compliance with applicable federal, state, and facility regulations and standards.
- Assist with survey readiness and maintain accurate clinical documentation.
- Review and monitor nutritional outcomes and develop interventions when necessary.
- Provide education and training to staff regarding nutrition, therapeutic diets, food allergies, and related topics.
- Work with the Dietary Department to ensure residents' nutritional needs and preferences are effectively addressed.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to nutrition and dining services.
Qualifications- Registered Dietitian (RD/RDN) preferred or required based on applicable state and facility requirements.
- Bachelor's degree in Dietetics, Nutrition, or a related field.
- Previous experience in a skilled nursing, rehabilitation, hospital, or long-term care setting preferred.
- Strong knowledge of clinical nutrition, therapeutic diets, and nutrition care planning.
- Experience with MDS, care plans, and regulatory requirements in long-term care preferred.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work effectively as part of an interdisciplinary team.
