Registered Dietitian (RD) to join our clinical team. This is an excellent opportunity for a compassionate nutrition professional who is passionate about improving the health, well-being, and quality of life of our residents.
The Registered Dietitian will play an important role in providing comprehensive nutritional care, collaborating with the interdisciplinary team, and ensuring residents receive individualized nutrition plans based on their clinical needs.
Responsibilities- Conduct comprehensive nutritional assessments for new and existing residents.
- Develop and implement individualized nutrition care plans.
- Monitor residents for weight changes, nutritional risks, and other nutrition-related concerns.
- Make appropriate recommendations regarding therapeutic diets, supplements, and nutritional interventions.
- Collaborate with nursing, MDS, medical providers, rehabilitation, and other members of the interdisciplinary team.
- Participate in care plan meetings and provide nutrition-related recommendations.
- Monitor the effectiveness of nutritional interventions and update care plans as needed.
- Maintain accurate and timely clinical documentation.
- Provide nutrition education to residents, families, and staff as appropriate.
- Assist with menu reviews and ensure nutritional needs and dietary requirements are appropriately addressed.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Maintain compliance with applicable federal, state, and facility requirements.
- Support survey readiness and maintain high standards of clinical nutrition services.
Qualifications- Registered Dietitian (RD/RDN) credential required.
- Bachelor's degree in Dietetics, Nutrition, or a related field.
- Current registration with the Commission on Dietetic Registration (CDR).
- Experience in a skilled nursing, long-term care, rehabilitation, or healthcare setting preferred.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work effectively as part of an interdisciplinary team.
- Compassionate, organized, and detail-oriented approach to resident care.
