The Enclave at Rye is a well-established Skilled Nursing Facility committed to providing high-quality, resident-focused care. We are seeking an experienced and detail-oriented MDS Director (RN) to lead our MDS/RAI process and support strong clinical outcomes and accurate reimbursement.
MDS Director (RN) Responsibilities

Oversee the MDS/RAI process, ensuring timely and accurate completion of all assessments

Coordinate with the interdisciplinary team to develop and update resident care plans

Ensure compliance with federal and state regulations

Monitor reimbursement accuracy under PDPM

Educate and support nursing staff on MDS and documentation processes

Participate in quality improvement initiatives and audits
MDS Director (RN) Qualifications

Active RN license (required)

Minimum 2 years of MDS experience in a Skilled Nursing Facility

Strong knowledge of PDPM, care planning, and regulatory guidelines

Excellent communication, assessment, and organizational skills

Leadership experience preferred
