Seeking a Recreation Therapist to join our Community Based Adult Day Health Care Center.

Job Description:

Responsible for planning and conducting group and individual recreation programs as assigned by the Director of Recreation
Responsible for documenting program attendance and participation
Facilitate independent activities for residents
Assist residents to and from recreation programs
All other tasks as assigned by the Director of Recreation
Work as part of the care plan team in determining therapeutic objectives and goals.
Create patient specific care plans
Other duties as required
