Wound Care Nurse / Infection Preventionist (RN) to lead wound care programs and support infection prevention and control efforts within our skilled nursing facility. This dual-focus role is vital to enhancing resident outcomes, promoting best practices, and ensuring compliance with clinical standards and regulatory requirements.
Wound Care Nurse / Infection Preventionist (RN) Key Responsibilities: - Lead and coordinate wound care assessment, treatment planning, and monitoring for residents
- Oversee the facility's infection prevention and control program, including surveillance, education, and reporting
- Collaborate with interdisciplinary teams (nursing, therapy, physicians) to support resident care plans
- Monitor infection trends and provide data analysis to guide prevention strategies
- Ensure compliance with CMS, DOH, and IP standards
- Provide staff education on wound care protocols and infection control best practices
- Maintain accurate documentation and ensure thorough clinical records
- Participate in quality improvement initiatives and survey readiness activities
Wound Care Nurse / Infection Preventionist (RN) Qualifications: - Current Registered Nurse (RN) license in New York State
- Experience in wound care and infection prevention in a skilled nursing, long-term care, or acute care setting
- Strong knowledge of clinical wound care standards and infection prevention protocols
- Excellent communication, organizational, and leadership skills
- Ability to work collaboratively with clinical and administrative teams
