Position Overview The Social Worker is responsible for addressing residents' psychosocial needs, assisting with discharge planning, and collaborating with the care team to ensure the highest level of resident support and satisfaction.
Key Responsibilities - Assess residents' social, emotional, and psychosocial needs
- Develop and maintain individualized care plans
- Participate in interdisciplinary care plan meetings
- Coordinate discharge planning and transitions of care
- Assist residents and families with community resources, home care services, and discharge arrangements
- Provide emotional support and counseling to residents and families
- Advocate for resident rights and overall well-being
- Maintain accurate documentation in compliance with facility policies and regulations
- Collaborate with nursing, therapy, and administrative teams to support quality resident care
Qualifications - Bachelor's Degree in Social Work (BSW) or related field required; MSW preferred
- Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred
- Strong communication and interpersonal skills
- Knowledge of discharge planning and care coordination
- Ability to multitask and work effectively in a fast-paced environment
- Experience with PCC and MDS 3.0 is a plus
