Cortlandt Healthcare in Cortlandt Manor, NY is a 120 bed sub-acute facility hiring a full time Physical Therapist Assistant. Hiring is done on site by our Director of Rehab and facility leadership, not by an outside recruiter. You will interview with the people you will actually work with.

- Your supervising PT is in the building. Not on the phone, not covering from another site. When something changes with a resident, you can walk down the hall and talk it through.
- The whole team is in the room. PT, OT, SLP, nursing, dietitian, respiratory, social work, and MDS build one plan together. You are included in it, not handed the end of it.
- New graduates are trained, not thrown in. Structured orientation, a named mentor, a gradual caseload ramp, and in house education. If sub-acute is new to you, that is expected and planned for.
- There is somewhere to go from here. Across 35 facilities we support falls reduction programs, restorative partnerships, wheelchair seating and positioning, and senior or lead PTA roles. [OPTIONAL: We also support PTAs pursuing PT school through flexible scheduling.]

What the Job Looks Like - Deliver physical therapy treatment according to the plan of care established by the supervising Physical Therapist
- Carry out therapeutic exercise, gait and transfer training, balance activities, and functional mobility training
- Adjust treatment within the parameters of the plan of care based on how the resident presents that day
- Report changes in status, response to treatment, and progress toward goals to the supervising Physical Therapist
- Document daily treatment notes accurately and on time, and contribute objective data to progress notes and recertifications
- Train residents, families, and caregivers on safe mobility, transfers, and assistive device use
- Partner with nursing on safe transfer status, fall prevention, and restorative programming
- Set up and maintain the therapy gym and equipment, and support student PTAs on clinical rotation
What You Need - Associate degree from an accredited Physical Therapist Assistant program
- Valid New York Physical Therapist Assistant license, or limited permit
- Willingness to work under the direction and supervision of a licensed Physical Therapist per the New York practice act
- Accurate, timely daily documentation
- Commitment to confidentiality, HIPAA, and Federal Resident Rights
Helpful, Not Required - Sub-acute, skilled nursing, or geriatric experience
- Familiarity with restorative nursing programs
- Experience with gait training equipment, NMES, or seating and positioning
New graduates and PTAs coming from outpatient, acute, or home care are encouraged to apply. We will get you up to speed.
