Cortlandt Healthcare in Cortlandt Manor, NY is 120 bed sub-acute facility and is hiring a full time Occupational Therapist to join our rehabilitation team. Hiring for this role is done on site by our Director of Rehab and facility leadership, not by an outside recruiter. You will interview with the people you will actually work with.

What You Will Do - Evaluate referred residents to determine medical status, functional ability, limitations, precautions, and rehabilitation potential
- Build individualized treatment plans with measurable, achievable goals based on each resident's capacity and tolerance, under physician direction
- Deliver skilled occupational therapy interventions focused on ADL performance, safety, and return to prior level of function
- C omplete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time
- Educate residents, families, and caregivers on therapy techniques, safety, and carryover strategies
- Collaborate with PT, SLP, nursing, dietitians, and social work to deliver patient centered care and support discharge readiness
- Recommend adaptive equipment, durable medical equipment, and environmental modifications
- Supervise Occupational Therapy Assistants and students in accordance with the state practice act
What You Will Need - Degree in Occupational Therapy from an accredited program
- Valid Occupational Therapist license and registration in the state of practice, or license eligible
- Ability to supervise Occupational Therapy Assistants in accordance with the state practice act
- Comfort with clinical documentation including evaluations, weekly progress notes, recertifications, and discharge summaries
- Commitment to confidentiality, HIPAA, and Federal Resident Rights
Preferred, Not Required - Experience in long term care, sub acute, or geriatric rehabilitation
- Prior experience in an interdisciplinary care model
- New graduates and therapists new to skilled nursing are encouraged to apply. We train.
